Terms of Use and Notices For APQC's OSBC Database (“Terms of Use”)

1. APQC's Open Standards Benchmarking CollaborativeSM ("OSBC") database consists of surveys, databases and data reports collected through APQC's Web site and APQC's Knowledge BaseSM (collectively, the "OSBC Database"). As a condition to accessing the OSBC Database, you agree to provide accurate survey information as requested by APQC. Your organization will own your specific data submitted to APQC. APQC will own the OSBC Database and any other information disclosed by APQC to your organization. APQC reserves the right to deny or limit privileges to the OSBC Database to any user without notice. By accepting the Terms of Use, you represent that you are an employee or and/or have authorization from the organization you are providing data to OSBC on behalf of to bind the organization to the Terms of Use, provide the information as requested by the APQC Web sites, and respond to surveys presented therein. For purposes of the Terms of Use, "you" means you and your organization. "Employee" is defined as a person employed full-time or part-time with non-temporary status at an organization. You must be eligible to receive a W-2 tax form or international equivalent from the organization you list as your employer while accessing the OSBC Database. OSBC Database users are not allowed at any time to represent and/or provide data for any other organization than their employer, unless a user has written permission from the organization. In the event a consultant or vendor is working on behalf of another organization, the consultant or vendor must have an employee of that organization provide and verify the data submitted to the OSBC Database.

2. Company-specific data entered by you to complete each survey and submitted to APQC is confidential and subject to APQC's privacy policy. The OSBC Database and any other information disclosed by APQC to you is confidential. The data collected and maintained in the OSBC Database by any such survey shall be owned by APQC and may be used by APQC in any manner subject to APQC's privacy policy. Information provided to APQC via the OSBC surveys will be used in accordance with the Benchmarking Code of Conduct.

3. The OSBC Database is intended to support the understanding, assessment, measurement, and change of internal processes within an organization. You may only use the OSBC Database on an individual, limited, non-commercial, and personal basis. Except as may be provided by special agreement or license between you and APQC, you may not modify, copy, distribute, transmit, display, perform, reproduce, publish, license, create derivative works from, transfer, or sell any surveys, data, information, products, or services obtained from the OSBC Database. Without the advance, expressed written permission of APQC, you may not use the OSBC Database in any commercial manner. You acknowledge that violation of this provision would cause irreparable harm to APQC not adequately compensable by monetary damages and, in addition to other relief, you agree that APQC may seek injunctive relief without necessity of posting bond to prevent any actual or threatened violation of this term.

4. APQC does not claim ownership of the materials you provide to APQC (including feedback and suggestions) or post, upload, input, or submit to the OSBC Database. However, by posting, uploading, inputting, providing, or submitting data, you are giving consent to APQC, in accordance with the APQC Benchmarking Code of Conduct, to use your submission in connection with the operation of its businesses (including, without limitation, the OSBC Database), including, without limitation, the license rights to: copy, distribute, transmit, publicly display, publicly perform, reproduce, edit, translate and reformat your submission; to publish your blinded data in connection with your submission; and to sublicense such rights to any supplier of an OSBC Database. APQC will not pay you any compensation with respect to the use of your submission. APQC is under no obligation to post or use any submission you may provide and APQC may remove any submission at any time in its sole discretion. By posting a submission you warrant and represent that you own or otherwise control all of the rights to your submission as described in these terms of use including, without limitation, all the rights necessary for you to provide, post, upload, input, or submit the submissions.

5. APQC provides the OSBC Database "as is" without warranty of any kind, and APQC hereby disclaims all warranties whether express or implied, oral or written, with respect to information comprising the OSBC Database including without limitation all implied warranties of merchantability, fitness for any particular purpose, title, or non-infringement. Changes are periodically made to the OSBC Database and to the information therein.

6. APQC's liability for damages to you for any cause regardless of the form of any claim or action shall not exceed the fees actually paid by you (if any) during the term of this agreement.

7. You agree that APQC may list your organization name as a participant of the OSBC Database program it provides to customers and prospective customers.

8. APQC may change these terms and conditions at any time by notifying you of any changes.

Instructions

  Research project structure
APQC OSBC research projects are designed to collect a wide variety of information about your entire organization, with a specific emphasis on the particular business entity for which the research is being collected.

Each survey is divided into a number of sections, separated by a block of introductory text. The first section contains questions about your organization as a whole. The second section contains general questions about your particular business entity. Each of the subsequent sections of the research project presents questions about specific processes and activities in your business entity. Each of these process-specific sections corresponds to APQC's Process Classification Framework.

Before each section containing process-specific questions, you are presented with a list of components of the PCF. This list identifies which components of the PCF are addressed by this particular section of the research project. The numbers in the list correspond to the PCF's unique numbering scheme. Questions within the section will address specific activities for the stated component of the PCF.

 

Navigation, saving results, and submitting a survey
The survey is presented in pages. Each page has at least one question on it. Each question on each page should be answered. A survey may also have a question that must be answered in order to proceed. Required questions are identified by the word "Required" in red letters.

Use the buttons at the bottom of each page to navigate throughout the survey. To avoid data loss, do not use the browser "Back" and "Forward" buttons to navigate through the survey. As you navigate through the survey using the "Back" and "Forward" buttons, your data are saved to APQC's secure servers.

The progress indicator at the bottom of each page displays a bar graph and a percentage that indicate your progress in navigating through the survey. The progress indicator does not account for completed questions, only for your relative position in the survey.

To exit the survey before you have completed all questions, click the "Save" button at the bottom of the survey page, then close your browser. The system will present you a link to your particular instance of the survey. Use that link to return to the survey at any time.

Once you have completed the survey, click the "Submit" button on the last survey page. By submitting your survey results, you will notify APQC that your data are ready for validation. You will not be able to create, review, update or delete your answers after you have submitted the survey, so ensure that you have accurately documented your answers before you submit.

 

Cookies, one browser
The APQC OSBC survey tool uses a "cookie" to associate you to a particular survey instance. Your cookie is unique to the computer you used to start the survey and is the only key that you have to access your data. Should you lose your cookie, you will be unable to complete the survey and will need special assistance from APQC. Do not delete your APQC cookies until after you have submitted your survey data.

Because this cookie is unique to the computer you used to start the survey, other users will be unable to contribute results and data to the survey unless they have physical access to the browser you used to start the survey.

JavaScript
The APQC survey application uses JavaScript for enhanced validation and usability features. In order to complete these surveys, you must use a browser that supports JavaScript, and you must have JavaScript enabled. Visit the Browser Setup page for more information.

 

Printing
To print a copy of the entire survey, you should use the "Preview" version of the survey. This view consolidates all of the questions associated with the particular survey onto a single printable browser page. No answers or personal data are contained on the "Preview" version of the survey.

As you navigate through the survey, you may print each page to record your answers. There is no facility to review all of your answers on one page.

 

Definitions
Each survey has a set of definitions associated with it that are unique to the process grouping covered by the survey. Throughout the survey, questions may have links to survey definitions.

Accurate results depend on a common understanding of the terminology used across survey respondents, so take special care to review the definitions in the glossary thoroughly. Common terms may be defined differently than you expect. When in doubt, contact APQC for guidance.

 

Reasonable numbers
The survey asks for a number of detailed pieces of information. If you do not have the exact information that a question asks for, please provide a reasonable approximation. If you cannot provide a reasonable approximation, please leave the answer blank. If you are not sure, contact APQC for guidance.

 

Reporting period
All questions in the survey, unless expressed otherwise, request data from the last complete fiscal year of your organization's or business entity's operations. Please be sure to provide the end-date of the fiscal year for which you are providing data. If you have any questions about the reporting period, please contact the OSBC help desk. Question numbers may skip
Occasional changes to the survey may result in the addition or removal of questions. Due to this activity, the question numbers in the survey may occasionally skip one or more numbers. Furthermore, some surveys will hide questions depending upon answers to previous questions. If you have any questions about the order of questions in the survey, please contact the OSBC help desk.
